Argus reporter Kath Skellon said she was driving from Newport towards Abergavenny just before 6pm and she saw the orange car that had left the road and gone down a steep bank.
She said the car seemed to have hit a tree and that the scene looked "very serious".
She added police cars were at the scene and officers were carrying out an investigations.

A man has died following the crash on the A449 yesterday.
The 82-year-old man was pronounced dead at the scene. A 79-year-old passenger and the 80-year-old driver were taken to hospital and both are stable.
There three men, all from the Cardiff area, were travelling in an orange Renault Captor.
East Area Support Unit Inspector Lee Ford said: "This is a busy stretch of road with lots of traffic on it at the time. It's a distinctive vehicle so I would hope that someone would have information that could help us . I would ask anyone who witnessed this incident to contact us, if they haven't already, or anyone who saw this vehicle beforehand to call us on 101 quoting log number 334 25/6/15".

A Wales Ambulance Service spokeswoman said two people have been taken to hospital - one to the Royal Gwent Hospital in Newport and the other to Nevill Hall Hospital in Abergavenny.
An air ambulance was called but did not land because it was not required.
UPDATE: 4.31pm
THE A449 is closed both ways from the Coldra Roundabout in Newport to the Raglan Interchange.
It follows a serious accident involving one vehicle on the road southbound, one mile outside Usk, at about 3.36pm. Three people have been injured.
Firefighters from Usk, New Inn and Ebbw Vale are dealing with the incident and diversions are in place. They were called at 3.49pm.
A Wales Ambulance Service spokeswoman said three ambulances, a rapid response vehicle and an air ambulance have been sent there.
A Gwent Police spokesman said motorists are asked to avoid the area because of congestion and anyone who might have seen the incident is asked to call 101 and quote log number 25/6/15.
