The eighth South Wales Health and Care Awards will take place to honour outstanding health and care workers.

The ceremony at Rodney Parade on December 4, in association with the University of South Wales, aims to recognise the resilience and dedication of these professionals.

Finalists for the Team of the Year Award have been announced, with teams demonstrating exceptional patient care, teamwork, communication, and cross-organisational collaboration.

The OAPL operates within general wards, supporting older patients, particularly those with dementia or delirium.

The team assists with mental health needs, facilitating their physical health treatment in a general ward.

They also manage violent or unmanageable patients, providing a familiar and reassuring presence.

Their teamwork extends to sourcing donations and providing personal care to patients, enhancing hospital stays.

St David's Hospice Care clinical nurse specialists

St David's Hospice Care clinical nurse specialists provide community-based hospice care for patients with life-threatening illnesses.

The palliative care clinical nurse specialists (CNS) team coordinates individualised care, working with GPs, district nurses, and other healthcare professionals.

The team offers holistic care to patients and families during their most challenging times, with families expressing gratitude for the team's supportive presence.

St Gwynllyw Development Centre

St Gwynllyw Development Centre, a learning development centre for adults with learning disabilities, has been nominated for the award multiple times.

The centre offers bespoke learning and development opportunities, fostering a safe and nurturing environment.

The team recently opened a café called Be Our Guest, run by the people they support, which has boosted confidence and skills.
