The athletes will participate on five sports-themed floats, dressed in their team tracksuits, and wearing their medals.
The Parade of the Heroes starts from outside the Ritz on Piccadilly at 1.30pm on Monday, and will travel to Piccadilly Circus before turning down Haymarket and into Trafalgar Square at about 2.20pm.
There's also a celebration in Trafalgar Square from 1pm until 3pm, featuring live footage of the parade, highlights from the Olympic and Paralympic Games and interviews with athletes from the past and present.
The event will be shown live on BBC.
